Dear Parents
The Department of Health is offering Combined Diphteria,Tetanus, Acellular Pertussis & Inactivated Poliovirus Vaccine Immunization for students of K05. This is a free service.
It is important you return the following information before 18th January 2013 to the School Nurse:
- CONSENT form if you would like the vaccination
- OR REFUSAL FORM
- ALL PREVIOUS ORIGINAL IMMUNIZATION RECORD CARD(S) SAFELY ATTACHED TOGETHER
The Department of Health will check all students immunization status and carefully assess.

We are now entering what is considered to be the peak season for the transmission of Influenza and Respiratory Viral Infections in Hong Kong, and there are some cases in our school. We have taken up additional measures to provide an utmost hygienic environment for our students. We also ask for your help to protect our students by stepping up precautionary measures at home.
